Assessment: Reviewing Orders
• Checks accuracy and completeness of Physician Order including:
- Patient Name
- DOB
- MRN #
- Date and Time order was written
- Provider Signature
- Check medications for right dose, route, frequency, and parameter if needed (Ask
proctor for parameters, write on piece of scratch paper.)
Implementation: Acquiring Medications (pyxis)
• Performs hand hygiene and accesses automated dispensing system for appropriate patient, verifying name, allergies, DOB and MRN
• Selects correct medications: 3 medications
- Expiration date for each medication
- Compares name of each medication to MAR
• Checks calculation based on math performed; proctor CANNOT confirm answer until conclusion of PBA 1st medication check Six rights of medication administration for each medication, compare to MAR - IVPB, IM/SQ, IV. Student must state, “I will document later” as the sixth right.
Implementation: Medication Administration
• Applies clean gloves • Selects appropriate injection site, verbalizes appropriate location/landmarks on body for intended use • Cleanses site (Injection pad) with alcohol for 10 seconds • Uses appropriate technique for IM or SQ and inserts needle at appropriate angle quickly and firmly; injects medication • Withdraws needle quickly, activates needle safety, places swab/gauze over site, applies gentle pressure • Assesses client’s IV site for signs of inflammation or edema
