NUR 148 Test 4 Ch. 2
1. NST
Answer procedures
-apply external fetal monitor (FHR transducer and toco) -observe monitor strip for 15x15 accels of the FHR baseline -may ask to push button connected to EFM every time feels fetus move-makes a mark on FM strip -fetal movement should correlate with FHT accels (Vibroacoustic stimulation may be used with NST to wake sleeping baby to decrease time required to obtain a reactive NST)
2. NST
Answer Results and Explanation
-reactive=elevation of FHR with fetal movement, adequate fetal oxygenation (criteria-2 accels in a 20 min test period)
-nonreactive=absence of accels of FHR -equivocal=inconclusive test results Reactive-well oxygenated and take mom off monitor Nonreactive or equivocal notify HCP immediately, inform pt we need follow up, ensure follow up care
3. CST
Answer Procedures
Contractions stimulated either by nipple stimulation or administration of Pitocin (most common)
4. CST 1 / 2
Answer Results and Explanation
-negative=no late decels with uterine contractions and this is good result meaning fetus well oxygenated
-positive=persistent and consistent late decels associated with at least half of contractions and "bad" result-implies fetus is poorly oxygenated
5. BPP
Answer Procedures
Ultrasound test assessing movement, muscle tone, breathing, amniotic fluid volume, heart rate pattern (NST)
6. BPP
Answer Results and Explanation
8-10=normal and CNS functioning and fetus not hypoxic and follow up per HCP recommendation 6=equivocal and preterm fetus repeat BPP within 24 hours 0-4=abnormal and admit and plan for delivery
- FHR early decels...
Answer -Uniform shape
-Lowest point at same time as peak of contraction
- FHR variable decels....
- / 2
Answer -vary in shape, duration, and intensity
