Evaluation of the Project:
A (1): Comparison of performance measures and SMART goal: To determine the success of this project, I compared the results of the two key performance indicators to the SMART goal and outcome statement. The HIP SMART goal was aimed to reduce the 30-day readmission rate for patients with Congestive Heart Failure (CHF) by 10% through implementing evidence-based clinical practice. This will involve comprehensive training for nursing staff in effectively managing CHF patients, disease-specific education for patients and families, medication management, and improved discharge planning.The project's first KPI was for 95% of the nursing staff to complete comprehensive training on managing CHF patients. The actual completion rate was 98%, surpassing the target.This indicates that the staff effectively implemented and well-received the training program, ensuring they are better equipped to manage CHF patients. These results contributed to the project's overall goal of reducing the 30-day readmission rate for CHF patients by 10%.The Second KPI was for 95% of CHF patients to receive specific education and improved discharge plans. Reviewing patient records, feedback, and discharge summaries confirmed that this target was met. This suggests that the educational materials and discharge processes were effectively integrated into patient care. Meeting both KPIs indicates the project was implemented successfully. The comprehensive training for staff and improved patient education and discharge processes likely contributed to reducing the 30-day readmission rate for CHF patients, achieving 2 / 3

the project's primary goal. This demonstrates the effectiveness of using evidence-based clinical practices in healthcare improvement projects.A (2): Efficiency in Project Implementation: By breaking my project down into small, manageable tasks, I stayed organized and efficient while working on it. I created a schedule and assigned each task to a specific day. This allowed me to focus on one task at a time and avoid becoming overwhelmed. I also created a list of resources to complete each task. This ensured I had everything I needed to complete each task and prevented me from wasting time searching for resources. One example is a streamlined electronic training module for nursing staff, which reduced the time required to complete comprehensive training on managing CHF patients by 20%. Further, it allowed the hospital to train more staff in a shorter period without compromising the quality of the training.Effectiveness of Project Implementation: The project was completed on time and within the budget. This is because I created a schedule and stuck to it. I also made sure only to purchase the resources that I needed. The project was also completed within the scope, which prevented scope creep and ensured that the project stayed on track. This is because I planned the project carefully and communicated with the stakeholders regularly to ensure that the project stayed on track. One example is the Health Improvement Project (HIP), which successfully achieved its goal of having 98% of nursing staff complete comprehensive training for effectively managing CHF patients by the first week of implementation. Further, it exceeded the initial KPI target of 95%, demonstrating the project's effectiveness in ensuring that most nursing staff were well-prepared to manage CHF patients.A (3): Project schedule comparison: The project was planned to be implemented between 8/1/2024 and 9/15/2024. The actual implementation was conducted for HIP before 9/20/2024.
